Shift: Day Shift (5x8 Hours)

Hours: 09:00 AM – 05:00 PM

Pay Rate: $40 - $45/hour

Position Overview

We are seeking a Licensed Master Social Worker (LMSW) to provide psychosocial support services for patients in an inpatient medicine and surgery setting. The ideal candidate will work collaboratively with interdisciplinary healthcare teams to support patient care, discharge planning, and access to community resources.

• Master's Degree in Social Work (MSW) required.
• Active New York State LMSW License required.
• Minimum 1 year of recent LMSW experience required.
• Hospital or inpatient experience preferred.
• Strong knowledge of psychosocial assessments and discharge planning.
• Excellent communication, counseling, and crisis intervention skills.
• Ability to work effectively with diverse patient populations and healthcare teams.

• Conduct comprehensive psychosocial assessments.
• Provide supportive counseling and therapeutic interventions.
• Develop and implement discharge plans for inpatient medicine and surgery patients.
• Coordinate referrals to community agencies, rehabilitation facilities, and support services.
• Collaborate with physicians, nurses, case managers, and other healthcare professionals.
• Assist patients and families in addressing psychosocial barriers to care and recovery.
• Maintain accurate and timely documentation in accordance with facility and regulatory requirements.
