Physical Therapist - PACE Program

Monday-Friday, 8 AM - 5 PM
Job Description
The Physical Therapist for the PACE Program is responsible for assisting the physical therapy needs of each participant, which includes conducting initial assessments for potential participants, annual assessments, and evaluations whenever there is a change in condition.This role collaborates with the interdisciplinary team, participants, and their family members to create, update, and implement individualized plans of care to enhance participants' quality of life.
Responsibilities- Conduct initial and annual physical therapy assessments for participants.
- Collaborate with the interdisciplinary team to develop and implement plans of care tailored to individual participant needs.
- Assist in educating and training PACE staff and volunteers to ensure participant safety and functional abilities during daily activities.
- Communicate effectively with participants, families, and team members regarding care plans and progress.
- Graduate from a CAPTE accredited Physical Therapy program.
- Currently licensed as a Physical Therapist in the state of Colorado.
- A minimum of 1 year of physical therapy experience; 2 to 3 years of home health or home-based physical therapy experience preferred.
- Valid driver’s license, reliable transportation, and maintained automobile insurance throughout employment.
- Current CPR certification.
