Minimum Requirements- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Kentucky physical therapy license required to start the assignment.
- BLS certification required from AHA or ARC.
- Experience providing physical therapy in a Home Health environment preferred.
- Current, valid driver’s license and reliable transportation to travel to patient homes.
- Ability to perform patient evaluations, develop treatment plans, and document according to home health agency standards.
- Electronic medical record proficiency preferred. Previous experience with home health documentation systems is preferred.
