Minimum Requirements- Graduate of an accredited Physical Therapy program (Doctor of Physical Therapy or equivalent)
- Current, active, and unrestricted Physical Therapist license in Illinois
- Minimum of 1 year of clinical experience; home health experience strongly preferred
- Knowledge of home health regulations, including Medicare guidelines and documentation standards (OASIS experience preferred)
- Ability to perform comprehensive patient assessments and develop individualized plans of care
- Current CPR/BLS certification (American Heart Association or equivalent)
- Valid driver’s license, reliable transportation, and willingness to travel within assigned territory
- Ability to work independently in a home-based setting and manage a flexible schedule
- Strong clinical decision-making, communication, and documentation skills
- Preferred: Experience with electronic medical records (point-of-care EMR), geriatrics, chronic disease management, bilingual skills, or prior home health/community-based care
