Job Description:
- Home Health RN responsible for ~2 SOCs/day 2 additional visits
- Productivity requirement: 30 points/week (must meet for guaranteed 40 hours)
- Patient Population: Geriatric
- Territory: Up to 60-mile radius from hub
Requirements:
- Minimum 1 year recent Home Health experience
- OASIS experience required
- HCHB EMR experience required
- Active RN license (in hand)
- BLS certification required
- First-time travelers not accepted
Additional Info:
- No on-call, No holidays
- Locals accepted at travel rate
- Traveler must be flexible with territory
- Charting must be completed by 9 AM next day
- Timely communication required
