Travel PT Home Health jobs in Modesto, CA and surrounding cities like Turlock, Riverbank, Oakdale, Salida, and Patterson offer a 13-week assignment with 8-hour days, Monday through Friday. You will visit 6 to 7 patients daily, focusing on follow-up visits and starts of care, helping clients regain independence and mobility in their homes.Responsibilities include assessing patient needs, developing individualized treatment plans, educating patients and caregivers, and collaborating with an interdisciplinary team to ensure optimal outcomes. A California PT license is required. Home health experience is preferred but not required; experience with the geriatric population is a plus.
