Requirements:
- Pharm D. Degree from an accredited School of Pharmacy.
- 5 years of retail pharmacy experience required. Pharmacy operations leadership experience preferred.
- Registered Pharmacist in the state of Indiana and must be in good standing with the Indiana Board of Pharmacy.
- Additional certifications may be required.
