Orthopaedic Clinical Liaison
The Orthopedic Clinical Liaison provides comprehensive patient centric care across the care continuum for orthopedic patients. The Liaison collaborates with the patient, multi-disciplinary team, and post-acute providers prior to admission to assess, plan, educate, advocate, and facilitate the services required to safely transition from the hospital at the time of discharge.Collaborative effort promotes quality outcomes to prevent unnecessary readmissions and complications during the recovery period. The Liaison is responsible for the development and maintenance of the Total Joint Program development; and assists with data collection to ensure quality outcomes are maintained and program effectiveness is sustained.
The program and the Liaison’s primary focus is patient and community education as well as patient satisfaction. This role serves as a resource for patients, families, providers and other staff members in the care of surgical patients.
Minimum Education- Graduate of an accredited school of nursing with BSN.
- 3 years of orthopaedic experience in hospital or clinic setting.
- 3 years of case management experience.
- Experience with program development and management preferred.
- Current Vermont RN license.
- Orthopedic Nurse Certification: ONC – National Association of Orthopedic Nurses.
- BLS Certification through American Heart Association.
- Valid Driver's License
- Demonstrates safe knowledge of nursing practice skills at all times based on theory and consistent with policy and procedure.
- Demonstrates advanced knowledge and practice of nursing.
- Strong written/verbal communication skills.
- Critical thinking and analytical skills.
- Teaching and coaching skills.
- Ability to effectively collaborate with providers & all levels of the organization
- Basic Microsoft Windows desktop application and navigation skills.
- Willingness and ability to utilize personal vehicle and insurance for travel.
- Knowledge of quality improvement methods, data analysis methods
Pay Range = $38.91 - $56.68
