Description
American Traveler is seeking a Home Health Physical Therapist with at least 1 year of recent experience to serve a geriatric patient population across a defined territory in the Livermore, Pleasanton, and Dublin area.
Details- Home health setting serving a geriatric patient population
- Territory covers the West Team area, including Livermore, Pleasanton, and Dublin
- Average caseload of 5–7 patients per day
- Day shift, Monday–Friday, 8:00 AM–5:00 PM (5x8-hour shifts)
- No on-call or weekend requirements
- Documents patient care using HCHB (Homecare Homebase) EMR
- iPad provided for documentation in the field
- Mileage reimbursed at the IRS standard rate
- Minimum 1 year of recent Physical Therapy experience required
- Home health experience preferred
- Current BLS certification required
- Experience with HCHB EMR required
- OASIS experience preferred
- PT license required if applicable
- Must be comfortable traveling within a territory of up to 60 miles from the hub/office
- Must be able to meet productivity point requirements
- Traveler is expected to remain flexible with territory coverage as needs shift
- Local candidates are accepted and eligible for the travel rate
- First-time travelers are welcome to apply
- Scrubs required in any color; jeans are not permitted
- A cell phone is not provided for this position
- 2 professional supervisor references from within the past 2 years are required for consideration
- A full 7-year work history with all gaps explained is required for consideration
