Schedule: Day Shift | 5x8 Hours

Position Overview

We are seeking a Transplant Social Worker for an outpatient transplant program in Washington. This role focuses on supporting pre- and post-liver transplant patients through psychosocial evaluations, caregiver coordination, and ongoing patient support.

• No weekends
• No holidays
• No on-call responsibilities
• No floating required
• Outpatient-based role with occasional inpatient evaluations depending on department needs
• Manager interview required prior to offer

• MSW required
• LICSW preferred
• Minimum 1 year of experience required
• Strong psychosocial assessment skills required

• Conduct psychosocial evaluations for pre- and post-liver transplant patients
• Verify caregiver support systems via phone communication
• Support transplant patients through care coordination and counseling
• Collaborate with multidisciplinary transplant teams
• Manage approximately 10 new patient evaluations weekly
