Job Details- Weekly Estimated Pay: $2,120 - $2,230
- Location: Scranton, PA (zip code 18508) — serving Luzerne County and Northeastern Pennsylvania
- Position Type: Travel, Home Health RN / Homecare RN
- Hours per Week: 40 (weekly hour guarantee: 32)
- Schedule: Monday–Friday, 8:00am – 4:30pm (8-hour shifts)
- On-Call/Weekend: Every 3rd weekend; 1 night on-call per week
- Dress Code: Scrubs (no specific color required)
- Mileage Reimbursement: Yes — reimbursed between patient visits
- Contract Duration: Through December 31, 2026
- Active Registered Nurse (RN) license in Pennsylvania
- Associate's Degree in Nursing (ADN) or higher
- Minimum 1 year of nursing experience; recent home health experience preferred
- Experience with HCHB (Homecare Homebase) is a plus
- Professional certification required
- Valid driver’s license and ability to travel within assigned territory
- Serve as RN case manager for an assigned territory, providing comprehensive home health nursing care
- Perform skilled routine visits, recertifications, discharges, and start/resumption of care
- Provide 100% patient care in the home setting—assessments, care planning, patient/family education
- Document accurately in electronic health records (EHR) and follow agency policies—HCHB experience preferred
- Collaborate with interdisciplinary team and other case managers to ensure continuity of care and coverage during days off
- Meet productivity expectations (30 points per week) and participate in scheduled on-call rotation and weekend coverage
