Position Overview:
"Navitas Healthcare, LLC" is seeking an experienced Home Health Occupational Therapist to provide individualized therapy services to patients in their homes. The OT will evaluate, develop, and implement treatment plans that improve patients' independence and quality of life while collaborating with interdisciplinary healthcare teams.
Primary Responsibilities:
- Conduct occupational therapy evaluations and assessments
- Develop and implement individualized treatment plans
- Assist patients in improving activities of daily living (ADLs)
- Provide therapy services for patients with neurological, orthopedic, and chronic conditions
- Complete OASIS documentation accurately and timely
- Educate patients and caregivers on treatment plans and home safety
- Monitor patient progress and modify treatment plans as needed
- Collaborate with physicians, nurses, and healthcare team members
- Maintain accurate documentation using EPIC and home health systems
- Travel to patient homes within assigned service areas
Required Qualifications:
- Active Oregon and Washington Occupational Therapy Licenses
- BLS Certification required
- Minimum 1 year of Skilled Home Health experience
- OASIS documentation experience required
- Reliable transportation required
- Strong communication and patient care skills
Preferred Qualifications:
- Experience with Home Health billing processes
- Knowledge of National Patient Safety Goals and Core Measures
- Experience with acute and chronic disorders
- Functional activities and rehabilitation experience
Patient Population:
- Dementia
- Parkinson’s Disease
- ALS
- Multiple Sclerosis (MS)
- Orthopedic Post-Operative Patients
- Post-Stroke Patients
Additional Information:
- First-time travelers accepted
- Productivity expectation: 30 points per week
- May visit up to 6 patients per day
- Mileage reimbursement provided
- Charting: EPIC Remote Client and Rover App
- Business casual attire or scrubs permitted
