Registered Nurse – Case Manager - 26-04539

We are seeking an experienced RN Case Manager to support discharge planning and care coordination in an acute care setting. The ideal candidate will have a strong background in case management and the ability to manage a high patient caseload efficiently.

Key Responsibilities:

• Coordinate patient care and discharge planning
• Collaborate with interdisciplinary teams to ensure appropriate care transitions
• Assess patient needs and develop individualized care plans
• Monitor patient progress and adjust plans as needed
• Communicate with patients, families, and healthcare providers
• Maintain accurate documentation in EMR systems

Patient Load:

• Approximate ratio: 1:20

Requirements:

• Minimum 2 years of acute care case management or discharge planning experience
• Active California RN license required (pending license accepted but must be active by start)
• First-time travelers accepted (subject to screening)
• Strong communication and organizational skills
