Travel Home Health Physical Therapist job in Medford, Oregon: Begin your assignment in Medford, OR and help patients regain mobility and independence in their homes. You will assess patient needs, develop and implement care plans, deliver therapeutic interventions, and educate patients and families on recovery strategies.Responsibilities include documenting visits, adapting treatment plans, and collaborating with healthcare professionals. You may visit up to 6 patients per day, with a 1:1 patient ratio. Proficiency in OASIS documentation and Homecare Homebase is required1.
Required qualifications include a current Oregon Physical Therapy license and at least one year of skilled home health experience.
