Social Worker Lead/MSW - Case Management

HOW YOU'LL MAKE AN IMPACT:
Job Summary:
- Leads functional teams and/or projects.
- Oversees others’ work but does not directly supervise.
- Recognized/serves as a social work expert.
- Utilizes a specialized depth and/or breadth of social work expertise.
- Works independently; receives guidance in only the most complex situations.
- Interprets internal or external issues; recommends solutions and/or best practices.
- Solves complex problems utilizing a broad perspective to identify solutions.
- Multi-disciplinary team member.
- Assesses and treats patients/clients with emotional and behavioral problems.
- Provides social work services to patients/clients with behavioral, social, financial/economic, and environmental problems.
- Interviews patients/clients and their families.
- Evaluates, and understands the social and emotional factors underlying patients’/clients’ illnesses; identifies their psychological needs, goals, and treatment preferences; coordinates a range of services; and facilitates individualized case management.
- Provides clinical counseling and individual therapy to help patients/clients make positive behavior changes.
- Assists patients/clients in treatment planning.
- Provides direction to clinical team for implementing treatment plans.
- Provides crisis intervention services to patients/clients and families.
- Advises family caregivers; provides case and care management or interventions designed to promote health and prevent disease.
- Provides patients/clients with information and counseling; makes referrals for other services.
- Maintains contact with patients/clients to assist in adjusting their needs through use of medical facilities or referral to outside supportive agencies.
- Performs discharge planning responsibilities.
- Participates in social and health programs in the community.
- Assists with staff orientation, education, and training; assesses needs, develops materials, and monitors/reports progress.
- May travel to patients’/clients’ homes to render services.
- Performs miscellaneous duties.
- Progression to the lead level is restricted based on business requirements.

WHAT YOU’LL NEED TO APPLY:
Required Education:
- Master’s degree in a mental health specialty (e.g., Master of Social Work, Master of Mental Health, Master of Mental Health Counseling, or Master of Marriage and Family Therapy) from an accredited university
Required Experience:
- One (1) year of relevant job-related post-master’s degree work experience.
