What You’ll Do
As a Home Health RN Case Manager, you will manage a caseload of patients and provide skilled nursing care in the home environment while developing your clinical expertise in a growing and dynamic setting.- Perform comprehensive patient assessments and develop individualized care plans
- Conduct initial and ongoing skilled nursing visits based on patient needs and physician orders
- Monitor and evaluate patient status, including physical, psychosocial, environmental, and caregiver factors
- Coordinate care with physicians, therapists, social workers, and other care team members
- Communicate changes in patient condition and recommend appropriate interventions
- Update and revise care plans as patient needs evolve
- Provide education and support to patients and caregivers
- Maintain accurate, timely clinical documentation in compliance with regulations and company standards
- Ensure adherence to HIPAA and patient confidentiality requirements
- Current, unrestricted Registered Nurse (RN) license in the applicable state
- Minimum of 1 year of nursing experience in a clinical setting
- Valid driver’s license, auto insurance, and reliable transportation
- Willingness to travel within the assigned territory (approximately 50%);
- Ability to sit, stand, bend, lift, and move intermittently
- Ability to lift 50–100 lbs as needed
