Travel Home Health RN – Forty Fort, PA | Homecare Registered Nurse (RN) opportunity serving Luzerne County patients. Join a dedicated home health team and deliver skilled nursing and case management in patients' homes across Forty Fort, Pennsylvania.
Position Details – Homecare RN (Travel)- Weekly Estimated Pay: $2,120 - $2,230 (competitive travel nursing pay)
- Location: Forty Fort, PA 18704 (Luzerne County) — Pennsylvania nursing jobs
- Employment Type: Travel / Home Health RN
- Hours per Week: 40 (Guaranteed minimum 32 hours/week)
- Shift: 8-hour shifts, Monday–Friday (8:00am–4:30pm)
- On-Call/Weekend: Every 3rd weekend; 1 night of on-call per week
- Duration: 39 weeks (through December 31, 2026)
- Dress Code: Scrubs (no specific color required)
- Mileage: Reimbursed between patient visits
- Active Registered Nurse (RN) license in Pennsylvania
- Associate's Degree in Nursing (minimum education requirement)
- At least 1 year of nursing experience; recent Home Health experience preferred
- Experience with HCHB (Homecare Homebase) is a plus
- Professional certification required
- Serve as RN case manager for a designated territory within Luzerne County
- Provide skilled nursing visits, recertifications, discharges, and start/resumption of care
- Deliver 100% patient-focused care to ensure high-quality home health outcomes
- Collaborate with other case managers and clinical staff to provide coverage and continuity of care
- Meet productivity expectations and maintain accurate, compliant documentation
- Guaranteed minimum hours and clear weekday schedule — ideal for work-life balance in travel nursing
- Mileage reimbursement between patient visits
