Position Overview
Community Hospital is seeking a compassionate and skilled Physical Therapist (PT) for a 13-week travel assignment supporting both outpatient and acute care rehabilitation services in Grand Junction, Colorado.
This position offers the opportunity to work with adult and pediatric patient populations in a collaborative multidisciplinary environment focused on helping patients achieve their highest level of independence and functional recovery.
Assignment Details- Assignment Type: Travel Contract
- Length: 13 Weeks
- Schedule: Full-Time
- Guaranteed Hours: 40 hours/week
- Overtime: Paid after 40 hours/week
- On-Call Requirement: None
- Holiday Requirement: None
Practice Setting
📍 Grand Valley Rehab at 12th Street & G RoadGrand Junction, Colorado
Coverage includes:
- Outpatient Rehabilitation
- Acute Care Services
Patient Population:
- Adult patients
- Pediatric patients
- Perform comprehensive patient evaluations and assessments
- Develop and implement individualized treatment plans
- Deliver evidence-based therapeutic interventions
- Monitor patient progress and modify treatment plans as needed
- Educate patients, caregivers, and families
- Coordinate care with interdisciplinary healthcare teams
- Complete timely and accurate documentation
- Submit appropriate billing and coding information
- Support quality care initiatives and patient outcomes
Qualifications
Required- Bachelor’s degree in Physical Therapy from an accredited program
- Current unrestricted Colorado Physical Therapist license
- Minimum 2 years of PT experience
- BLS Certification (must be obtained within 90 days of hire)
