Position Overview
We are seeking experienced and compassionate Registered Nurses (RNs) to perform in-home visits across all NYC boroughs. As a Fee-for-Service Registered Nurse, you will play a vital role in patient care, assessments, and care coordination—helping clients live safely and comfortably in their homes.
Compensation (Per Diem Rates)- Initial Assessment Visit: $130
- Reassessment Visit: $100
- Resumption of Care Visit: $100
- Supervisory Visit: $80
- Conduct initial and follow-up in-home nursing assessments
- Develop and update individualized care plans
- Educate patients and families on disease management and self-care
- Supervise and instruct caregivers and home health aides as needed
- Complete timely and accurate documentation in compliance with regulations
- Communicate effectively with interdisciplinary team members
- Active New York State Registered Nurse (RN) license
- 1–2 years of experience in CHHA or long-term care preferred
- Strong clinical assessment and documentation skills
- Professional liability insurance is required
- Reliable transportation and willingness to travel between boroughs
