Job Details- Weekly Estimated Pay: $2,120 - $2,230
- Employment Type: Travel Contract (Travel Nurse / Travel RN opportunity)
- Location: On-site in Baton Rouge, LA and surrounding parishes
- Duration: 13 weeks (April 20, 2026 – July 18, 2026)
- Shift: Monday–Friday, 8:00am–4:30pm (8-hour shifts)
- Hours per Week: 36 (guaranteed)
- On Call: Required every 5th weekend (on-call rotation)
- Dress Code: Scrubs (any color except neon)
- Mileage: Reimbursed between patient visits
- Minimum 1 year of hospice nursing experience (Hospice RN / home hospice experience preferred)
- Associate Degree in Nursing (ASN) required
- Active Registered Nurse (RN) license in Louisiana
- Experience with HCHB (Homecare Homebase) preferred — familiarity with home health EHR and clinical documentation is a plus
- Strong clinical assessment, patient-centered care, and communication skills
- Provide compassionate hospice and home health care in patients' homes across Baton Rouge and surrounding parishes
- Complete interdisciplinary group (IDG) notes, recertifications, admissions, and routine patient visits
- Meet productivity expectations (40 points per week) while maintaining high-quality patient care
- Collaborate with interdisciplinary teams (physicians, social workers, aides) to ensure optimal patient outcomes
- Maintain accurate, timely clinical documentation in HCHB or other home health EHR systems
- Participate in an on-call rotation every 5th weekend to support patient needs
