Minimum Requirements:
- Pharmacist License for KY or Compact as applicable
- Applicable recent experience in this work setting
- Minimum 1 Year of experience
- Commitment to quality care and adherence to client legal and protocols
- Certifications: EPIC experience
- Equipment/Procedures: Medication Reconciliation for discharge medication list; Antimicrobial Stewardship; Pharmacokinetic Dosing Management/Monitoring; Anticoagulation Dosing Management/Monitoring
