Location: Grand Junction, CO
Assignment Length: 8 Weeks
Shift: Variable Night Shifts
Shift Length: 12 Hours

Guaranteed Hours: 36 Hours/Week

Compensation
  • Local Rate: Up to $40/hr (based on experience)
  • Travel Rate: Up to $49/hr (based on experience)

Position Overview

"Navitas Healthcare, LLC" is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join a dedicated long-term care and skilled nursing team in Grand Junction, Colorado. The ideal candidate will possess strong clinical and patient care skills, with experience in long-term care, rehabilitation, or post-acute settings preferred.

This position requires flexibility for night shifts and the ability to thrive in a fast-paced healthcare environment while delivering exceptional resident-centered care.

Key Responsibilities
  • Provide direct nursing care in accordance with individualized care plans and physician orders
  • Administer medications and treatments safely and accurately
  • Monitor, assess, and document resident conditions and report any significant changes
  • Perform routine nursing procedures including wound care, vital signs, and basic assessments
  • Assist residents with activities of daily living (ADLs) while ensuring dignity, comfort, and safety
  • Collaborate with nurses, providers, caregivers, and interdisciplinary teams to support quality patient outcomes
  • Maintain accurate and timely clinical documentation
  • Adhere to infection prevention, safety, and regulatory guidelines
  • Support care planning initiatives and resident-focused treatment goals
  • Respond promptly and professionally to resident needs during assigned shifts
Qualifications
  • Active Licensed Practical Nurse (LPN) license in the state of Colorado required
  • Graduate of an accredited Practical Nursing program
  • Previous experience in long-term care, skilled nursing, rehabilitation, or post-acute care preferred
  • Current BLS/CPR certification preferred
  • Strong clinical, assessment, and documentation skills
  • Ability to work independently and collaboratively within a healthcare team
  • Excellent communication and interpersonal skills
  • Compassionate, dependable, and patient-focused approach to care
  • Ability to manage multiple patient assignments efficiently in a fast-paced environment
