The Admissions Registered Nurse Home Health, at the request of the Intake Department, provides nursing services to patients as ordered by the physician. The nurse is a specialist in assessing the patient’s condition and developing a plan of care to improve, monitor, and document the course of treatment prescribed by the physician for the patient’s diagnosis.
The plan of care is developed to reduce symptoms, educate the patient, gain patient’s compliance to the prescribed plan of care, and assist the patient in becoming independent with self care when capable, based on the individual needs of the patient.

Additionally, the nurse communicates with all members of the care team to insure proper coordination of care.

Primary Job Responsibilities
  • Evaluates the patient's appropriateness for Home Health services, considering regulatory compliance requirements and agency protocols.
  • Obtains Treatment authorization and admit patients to the agency.
  • Performs a comprehensive clinical evaluation.
  • Informs the physician of patient needs and obtain orders for any required changes.
  • Initiates the plan of care.
  • Teaches, supervises, and counsels the patient and family members regarding the nursing care needs and other related problems of the patient at home.
  • Performs any direct care necessary per physician orders.
  • Ensures patient is referred to appropriate agencies for continued supplemental services.
  • Completes Home Health OASIS assessment appropriate for patient visit timepoint.
  • Documents all activities in the Electronic Medical Record per agency policy.
  • Distributes a written start of care report to all care team members.
  • Meets established productivity targets by regularly checking in with Supervisor regarding any scheduling changes or additional assignments needed.
Requirements
  • Graduate of an accredited school of nursing
  • Current RN license in good standing to practice in the Commonwealth of Pennsylvania
  • Maintains clinical and professional competence
  • CPR certified
  • Driver's License and Car Insurance in good standing
  • Reliable vehicle
  • Possesses the ability to follow oral and written instructions
  • Possesses the ability to communicate effectively with all members of the care team
  • Possesses good organizational skills
  • Proof of COVID-19 Vaccination or an approved Medical or Religious exemption
  • Annual Influenza vaccination or an approved Medical or Religious exemption
