🏥 Job Summary:
A premier healthcare facility in western Colorado is seeking an experienced Outpatient Physical Therapist to join their rehabilitation team due to increased patient volume. The ideal candidate will evaluate patients, create individualized treatment plans, and help improve mobility, strength, balance, safety, and overall function.
âś… Must-Have Skills:
âś” Active Colorado PT License (DORA) or Compact License for Coloradoâś” Strong outpatient rehabilitation experience
âś” Excellent patient evaluation and treatment planning skills
âś” Ability to educate patients and caregivers on home exercise programs
âś” Strong communication and teamwork abilities
đź“Ś Experience Required:
✔ Minimum 3 years of Physical Therapy experience✔ 3–5 years preferred
âś” Outpatient therapy experience strongly preferred
🔑 Key Responsibilities:
- Evaluate patients and develop individualized treatment plans
- Implement therapy programs to improve strength, ROM, balance, and function
- Educate patients and caregivers on home exercise and recovery plans
- Maintain accurate patient documentation in EPIC EMR
- Collaborate with physicians and interdisciplinary healthcare teams
- Monitor patient progress and modify treatment plans as needed
- Ensure patient safety and quality care standards are maintained
