Licensed Practical Nurse – Long Term Care (Night Shift) - LPNLTC 0520 RR#03

Schedule: 36/48 Hours per Week

Job Summary

A compassionate and reliable Licensed Practical Nurse (LPN) is needed to provide high-quality care in a long-term care/skilled nursing setting. This role involves supporting patient assessments, administering medications, monitoring resident health, and assisting in the implementation of individualized care plans to ensure comfort and safety.

• Provide direct resident care in a long-term care environment
• Assist with patient assessments and monitor health status
• Administer medications and IV therapy as prescribed
• Maintain accurate and timely clinical documentation using EMR systems (Matrix)
• Support and implement individualized care plans
• Monitor and report changes in patient condition to the care team
• Ensure adherence to patient safety, infection control, and facility protocols
• Collaborate with interdisciplinary teams to deliver quality care

• Active North Dakota LPN License
• IV Certification
• Basic Life Support (BLS) Certification
• Ability to work rotating night shifts and weekends

• Experience in Long-Term Care (LTC) or Skilled Nursing facilities
• Familiarity with Matrix EMR system

• Strong clinical and patient care skills
• Effective communication and teamwork
• Medication administration and monitoring
• Attention to detail and accurate documentation
• Ability to manage workload in a high patient ratio environment (1:24)
