Minimum Requirements- Master's (MPT) or Doctor of Physical Therapy (DPT) degree from an accredited program.
- Active Colorado Physical Therapy license required to start the assignment.
- Home health experience with OASIS documentation required.
- Experience performing comprehensive patient evaluations, developing measurable plans of care, and documenting skilled necessity for reimbursement.
- Proven ability to provide skilled PT interventions in home settings, including therapeutic exercise, gait and balance training, transfer and stair training, and assistive device training.
- Experience educating patients and caregivers on home exercise programs, fall prevention, energy conservation, and caregiver transfer techniques.
- Experience coordinating with interdisciplinary teams and communicating with physicians regarding plan of care and discharge planning.
- Daily travel to patient homes within an approximately 50-mile service radius. Ability to travel to multiple locations within a shift required.
- Preferred: Experience with HCHB electronic documentation system.
- Certifications: BLS from AHA or ARC may be required.

- Shift Name: Days.
- Shift Details: Days 0800-1600.
- Work Days Per Week: Monday through Friday, five days per week. No call.
- Travel Requirement: Daily travel to patient homes within the agency service area; may visit multiple patient locations per shift.
- Dress Code: Scrubs in blue or black.

- Education Requirement: Minimum Master’s degree.
