Job Overview: We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) for a night shift Long-Term Care assignment. The role involves providing direct nursing care to residents in a Skilled Nursing Facility while ensuring patient safety, comfort, and high-quality care.

Assignment Details:

• Specialty: Long-Term Care (LTC)
• Shift: Night Shift (Variable)
• Shift Length: 12 hours
• Guaranteed Hours: 36 hours per week
• Contract Duration: 8 weeks

Key Responsibilities:

• Provide direct patient care under RN or physician supervision
• Administer medications and treatments as prescribed
• Monitor resident conditions and report any changes
• Assist with admissions, discharges, and assessments
• Maintain accurate documentation and charting
• Collaborate with interdisciplinary healthcare teams
• Ensure compliance with safety and infection control standards
• Support residents with daily care needs while maintaining dignity

Required Qualifications:

• Active Colorado LPN License
• Strong clinical and communication skills
• Ability to work night shifts and adapt to scheduling needs

Preferred Qualifications:

• Experience in LTC, SNF, or post-acute care settings
• BLS Certification
