Job Description:
Job Title: Travel Occupational TherapistJob Type: 13 Weeks Contract
Shift Schedule: Monday-Friday, 8:00 AM-4:30 PM
Coverage Areas: Oakland County, Wayne County, Livingston County, Washtenaw County, Genesee County, Macomb County (Primary area: Wayne County, Troy, and Utica)
Mileage Reimbursement: Mileage reimbursement is provided between patient visits (from the first patient to the last patient).Pay Range: $52/hr to $58/hr
Job Responsibilities:
- Perform discipline-specific evaluations to determine the need for occupational therapy services.
- Develop and implement an occupational therapy plan of care.
- Train patients in ADLs.
- Provide caregiver education.
- Complete accurate documentation.
- Conduct reassessments of home health patients.
- Perform admission/comprehensive assessments when no skilled nursing services are ordered and therapy services are included in the referral.
Must Have Qualifications/Experience:
- Graduate of an accredited occupational therapy program.
- Previous home health experience and familiarity with OASIS documentation, home health regulations and Conditions of Participation, functional assessment, fall prevention, ADL/IADL training, home safety evaluations, adaptive equipment recommendations, and care coordination with other disciplines.
- HCHB (HomeCare HomeBase EMR) experience.
Type of Certification Required:
- Current CPR/BLS certification.
- Unrestricted Occupational License in the state of practice.
Dress Code: Navy blue scrubs or slacks with a navy blue polo-style shirt.
