Schedule: Day Shift | 5x8s | 8:00 AM – 4:30 PM

Position Overview

We are seeking an experienced Home Health Physical Therapist for a travel assignment serving East Portland and surrounding areas. This role is ideal for clinicians who are passionate about providing individualized patient care in home-based settings while helping patients improve mobility, independence, and overall quality of life.

The Physical Therapist will manage a daily patient caseload, complete home visits, and collaborate closely with interdisciplinary healthcare teams to develop and implement effective treatment plans.

• Provide skilled physical therapy services in home health settings
• Evaluate patients and develop individualized treatment plans
• Perform functional mobility assessments and therapeutic interventions
• Educate patients and caregivers on exercises, safety, and home programs
• Complete timely and accurate Oasis documentation
• Coordinate care with physicians, nurses, and case managers
• Maintain productivity expectations while ensuring quality patient care

• Minimum 1 year of skilled Home Health Physical Therapy experience required
• Oasis experience required
• Active Physical Therapy license required
• BCLS certification required
• EPIC experience preferred
• Strong communication and documentation skills
• Ability to work independently in the field
