• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program.
• Active NC Physical Therapy license required prior to start of assignment.
• BLS certification required (AHA or ARC).
• Minimum of 1 year of Home Health physical therapy experience required.
• Experience with HCHB (Homecare Homebase) documentation workflow preferred.
• Proficiency in evaluations, reassessments, discharges, and supervision of PTA required.
• Driver's license and proof of auto insurance required. Ability to drive regional caseloads; average travel 60–80 miles per day.
• Comfort using EPIC Rover (view only), Focura for uploads, and company-issued smartphone for documentation.

• Schedule notes: No weekend rotation, no on-call, no major holiday coverage required. PTO/return to office requires manager approval.
