- Contract Duration: 13 weeks
- Shift Duration: 8-hour shifts
- Schedule: Monday through Friday, 8:00 AM to 5:00 PM, with on-call coverage and weekend rotation; at least 1 weekend per month
- Hours Per Week: 40
- Estimated Weekly Pay: $1,760 - $1,850
- Productivity Expectation: 30 points per week
- Active West Virginia RN license required
- At least 1 year of registered nursing experience required
- Home health experience required
- Experience with HCHB and OASIS strongly preferred
- Strong assessment, documentation, and care coordination skills
- Ability to work independently and respond effectively in an on-call setting
- Current RN credential and a commitment to safe, patient-centered care
- Observe, assess, plan, implement, and evaluate nursing care for well, chronic, and acutely ill individuals
- Coordinate with physicians, care teams, and other healthcare professionals to support patient outcomes
- Monitor medications, complete documentation, and support safe medication administration practices
- Complete daily, monthly, quarterly, and annual nursing documentation as required
- Maintain accurate nurse’s notes and ensure timely reporting when a patient’s condition changes
- Coordinate appointments, follow-up care, and communication with inpatient nursing and discharge teams
- Support and supervise LPN/LVN staff and train unlicensed staff on person-specific health and safety needs
- Help maintain equipment, furnishings, and supplies in accordance with infection control standards
