What You’ll Do- Manage a consistent caseload of patients in their homes
- Perform comprehensive assessments and develop individualized care plans
- Provide hands-on skilled nursing care and patient education
- Monitor progress and adjust care plans to ensure the best outcomes
- Collaborate with physicians, therapists, and the broader care team
- Build meaningful relationships with patients and families during critical moments in their healthcare journey
- Active, unrestricted RN license
- At least 1 year of nursing experience (home health experience is a plus, not a must)
- Strong clinical assessment and critical thinking skills
- A valid driver’s license, reliable transportation, and auto insurance
- Willingness to travel within your assigned territory
- Sit, stand, bend, lift and move intermittently and be able to lift 50–100 lbs.
