Description
American Traveler is seeking a Home Health Physical Therapist with at least 1 year of skilled home health experience for a 13-week contract covering West Portland, OR and surrounding areas.
Details- Home health setting serving West Portland, OR and surrounding areas
- 1:1 patient ratio with up to 6 patient visits per day; 30 productivity points is standard
- 8-hour day shifts, Monday–Friday (4x10 and 3x12 schedules cannot be accommodated)
- No weekend or on-call requirements
- Travelers may be expected to float between facilities within the state
- Charting is completed via Epic Remote Client (laptop) and Rover app (phone), with OASIS documentation
- Travel radius of up to approximately 60 miles per day, with up to 60–90 minutes of total drive time
- Minimum 1 year of skilled home health PT experience required
- Active Oregon PT license required (license required if applicable)
- Current BLS certification required
- Epic EMR experience required
- OASIS experience required
- Must have reliable personal transportation
- Primary COVID-19 vaccination series plus one booster required
- At least 2 supervisory references covering 1 year of employment within the last 3 years in a comparable specialty or setting required for consideration
- SSN and DOB required for consideration
- Open to first-time travelers
- Local candidates are accepted; candidates living within 50–75 miles of the facility are considered local and are eligible at the same rate
- Former permanent staff must wait at least 3 months before returning as a traveler, and may not be within 50 miles of the submitting facility
- Business casual attire required; scrubs are permitted in any color, but jeans are not allowed
- Scheduling is designed to group clients geographically to minimize drive and windshield time
- RTO requests should be discussed prior to submission; minimal RTO is preferred
- Floating between facilities within the state is expected; refusal to float may result in assignment termination
