Location: Tallahassee, FLSchedule: Full-time | Days (Rotating Weekends)
Working Conditions- Acute care hospital environment
- Day shift: 8:00 AM – 4:30 PM EST
- Rotating weekends (1 weekend per 4 weeks)
- Fast-paced, collaborative environment
- Hourly Pay: $31.94 – $43.92/hr depending on experience
- Shift Differentials: Evening +$2.50/hr, Weekend +$2.00/hr (stackable up to $4.50/hr)

About the Opportunity
Join a 288-bed acute care hospital providing comprehensive healthcare services, including emergency care, NICU, stroke center, cardiac services, and rehabilitation. With a strong focus on patient-centered care and operational excellence, this hospital offers a dynamic environment for experienced case management professionals.
As a Case Manager RN, you will play a key role in coordinating patient care, ensuring appropriate resource utilization, and facilitating safe and efficient patient transitions across the continuum of care.
Key Responsibilities
Care Coordination & Case Management- Perform comprehensive assessments of patients’ medical and psychosocial needs
- Develop and implement individualized case management and discharge plans
- Coordinate care across interdisciplinary teams to ensure optimal outcomes
- Evaluate admissions for medical necessity and appropriate level of care
- Conduct utilization reviews and communicate with third-party payers
- Monitor patient throughput and address barriers to care
- Ensure appropriate use of hospital resources without compromising quality
- Act as a liaison between physicians, patients, families, and external agencies
- Lead interdisciplinary team meetings to coordinate patient care plans
- Educate patients and families on post-discharge care and available resources
- Ensure compliance with regulatory standards and hospital policies
- Document case management activities and patient progress accurately
- Identify process improvement opportunities and support quality initiatives
Qualifications
Experience:
- Minimum 3 years of recent Case Manager experience in acute care (within the last year)
- OR 3 years of experience in Med/Surg, Tele, Neuro, ICU, PCU, or ED
- Home health or insurance experience acceptable with required acute care experience
Education:
- Associate Degree or Diploma in Nursing (Required)
- Bachelor’s Degree in Nursing (Preferred)
Licensure & Certification:
- Active Florida RN License or Compact License (FL license required within 90 days if using Compact)
- Case Management or Utilization Review Certification (Preferred)
