Position Overview
"Navitas Healthcare, LLC" is seeking a dedicated and compassionate Travel Licensed Practical Nurse (LPN) to join a Skilled Nursing Facility (SNF). In this role, you will deliver high-quality patient care, ensuring residents’ comfort, safety, and well-being while adhering to established care plans.This is an excellent opportunity for nurses who thrive in dynamic environments and are passionate about patient-centered care.
Key Responsibilities- Provide direct patient care in accordance with individualized care plans
- Administer medications and treatments as prescribed
- Monitor patient conditions and promptly report changes to the RN or physician
- Assist residents with activities of daily living (ADLs), including hygiene, mobility, and feeding
- Maintain accurate, timely, and compliant patient documentation
- Follow infection control policies and safety procedures
- Collaborate with interdisciplinary healthcare teams to enhance patient outcomes
- Active LPN license (state or compact)
- Minimum 1–2 years of experience in a Skilled Nursing Facility (SNF) or similar setting
- Strong clinical and patient care skills
- Ability to adapt quickly in a travel assignment environment
- Diploma or Certificate in Practical Nursing from an accredited institution
- Basic Life Support (BLS) certification (AHA preferred)
- COVID-19 vaccination documentation (if required by facility)
- Valid government-issued ID
- Background in long-term care, rehabilitation, or SNF settings
- Experience with Electronic Medical Records (EMR) systems
