Minimum Requirements- Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
- Active Wisconsin Physical Therapy license or multi-state license valid for Wisconsin is required to start the assignment.
- BLS certification from the American Heart Association or American Red Cross may be required.
- Experience working in skilled nursing facilities preferred. Candidates with experience in post-acute care, long-term care, or geriatric rehabilitation are preferred.
- Demonstrated competency with documentation systems commonly used in SNFs and experience with resident-focused care planning preferred.
