Duties:Job Summary: The Integrated case Manager for Population Health is an interdependent member of the patient-centered care team or treatment team responsible for the collaborative practice of assessment, planning, facilitation, care coordination, evaluation
Requirements-- Minimum 5 years of case management experience
- Recent 2 years of recent acute inpatient case management experience (REQUIRED)
- Strong experience in discharge planning
- Valid Michigan driver's license and insured vehicle
License & Certification : RN license required
