- Recent acute care/hospital Case Management experience required
- Must have:
- Midas experience
- Meditech experience
- Preferred:
- Complex discharge planning experience
- Works alongside RN Case Managers and Social Work Case Managers
- Patient ratio approximately 25:1 CM
- Travelers may alternate between units as needed
- EMR systems used: Meditech & Midas
- Strong discharge planning and coordination skills important
- Travelers expected to follow facility scheduling policies
- Friendly, adaptable team-player attitude preferred
