- Role: Registered Nurse (RN) – Case Manager
- Schedule: 3x12-hour shifts (36 hours/week)
- Shift: Day shift with weekend rotation
- Duration: 13 weeks
- Start Date: ASAP
- On-Call: None
- Coordinate and manage patient care plans across the continuum
- Support both inpatient and outpatient case management needs
- Collaborate with interdisciplinary teams to ensure optimal patient outcomes
- Assist patients and families with care planning, discharge needs, and resource coordination
- Minimum 3 years of RN Case Management experience (hospital setting required)
- Epic EMR experience – REQUIRED
- Active RN license (state of Colorado or compact)
- Graduate of an accredited nursing program
- Flu vaccine required (no exemptions)
- No call required
- Must live 75+ miles away from the facility to qualify as a traveler
