Hospice RN Travel Assignment in Grand Junction, CO — 12-week travel nursing opportunity for a compassionate Hospice Registered Nurse with an active Colorado RN license. Join a collaborative hospice and palliative care team delivering patient-centered end-of-life care in home and facility settings.
Travel Nurse Job Details- Weekly Estimated Pay: $1,870 – $1,970
- Location: Grand Junction, CO 81506 (Colorado hospice jobs, Grand Junction healthcare)
- Assignment Duration: 12 weeks (April 29, 2026 – July 22, 2026)
- Schedule: Monday–Friday, 8:00 AM–5:00 PM
- Guaranteed Hours: 40 hours per week
- Employment Type: Travel Nurse / On?Site Hospice RN
- Active RN license in Colorado (required)
- Minimum 1 year of nursing experience; hospice or palliative care experience preferred
- Strong clinical assessment, symptom management, and critical thinking skills
- Excellent communication, patient and caregiver education, and teaching abilities
- Ability to work independently in home hospice and collaboratively within an interdisciplinary team
- Reliable transportation and comfort providing care in homes, nursing facilities, assisted living, and group homes
- Provide compassionate, patient-centered hospice and palliative care across home and facility settings (home hospice nurse, hospice care nurse)
- Manage an assigned caseload and lead the interdisciplinary team for coordinated care
- Assess patient needs, monitor and manage complex symptoms, and implement individualized care plans
- Educate patients and caregivers on goals of care, medication management, and comfort-focused interventions
- Provide emotional support and guidance to patients and families during end-of-life care
- Document visits and clinical updates accurately in the medical record and communicate with the care team
- Participate in occasional weekend and on-call coverage as needed
