Required Education, Credential(s) and Experience:
- Education: Doctorate Degree Pharmacy; Post graduate Year 1 Residency or equivalent experience (at least 3 of clinical pharmacy practice) OR post graduate year 2 residency in Oncology or equivalent experience (at least 3 of oncology practice)
- Credential(s): Licensed Pharmacist, Board Certified Oncology Pharmacist
- Experience: Required: Three years of hospital experience and completion of clinical training and three years of training in oncology.
