Responsibilities
Help clients get started with services (intake + coordination with care team)Build relationships with hospitals, rehab centers, communities, and senior networks
Generate qualified client referrals for home care services
Meet with families to guide them through the admission process
Represent our agency at community events, networking groups, and local outreach opportunities
Maintain ongoing relationships with referral sources
