Physical Therapist - Phoenix, AZ - PTDS

Contract Duration: 13 Weeks

Shift: Days | Shift Length: 8 Hours | Guaranteed Hours: 32 Hours/Week

Position Overview:

Navitas Healthcare, LLC is seeking Physical Therapist – Skilled Nursing Facility for an exciting Travel or Local job in Phoenix, AZ.

• Evaluate patients and develop individualized physical therapy treatment plans
• Provide therapeutic exercises, mobility training, and rehabilitation services to patients recovering from injury, illness, or surgery
• Monitor patient progress and adjust treatment plans as needed to achieve rehabilitation goals
• Educate patients and family members on therapy programs, exercises, and injury prevention techniques
• Collaborate with physicians, nurses, and interdisciplinary healthcare teams to coordinate patient care
• Maintain accurate and timely documentation of patient treatments, progress notes, and discharge planning
• Assist patients in improving strength, balance, flexibility, mobility, and functional independence
• Ensure compliance with facility policies, patient safety standards, and healthcare regulations
• Participate in care planning meetings and contribute to quality patient outcomes

• Active Physical Therapist (PT) license in the state of Arizona or compact eligibility
• Graduate of an accredited Physical Therapy program
• Current CPR/BLS certification preferred
• Minimum of 1 year of recent Physical Therapist experience preferred
• Skilled Nursing Facility (SNF) or rehabilitation experience required
• Strong communication, patient care, and clinical assessment skills
