Physical Therapist Home Health in Baton Rouge, Louisiana. This temporary assignment involves performing patient evaluations and providing therapeutic services in a home health setting. The role requires direct patient care, supervision of therapy assistants, and coordination of treatment plans to support patient recovery and well-being.
Job Details:
- Schedule: Days, 8 hours per day, minimum 40 hours per week.
- Required Qualifications: Active physical therapy license specific to Louisiana; 2 years of physical therapy experience preferred; current CPR certification; reliable transportation, valid driver's license, agency-required liability insurance; familiarity with computer systems including email, internet, and online HR services.
- Preferred Qualifications: Experience in home health setting and working with home health EMR systems such as HOMECARE HOMEBASE/POINTCARE.
- Additional Information: The assignment lasts approximately 13 weeks, with a schedule from Monday to Friday, 8:00 AM to 5:00 PM. No weekend rotations or on-call duties are required. The contractor will serve within a 50-mile radius from the Baton Rouge care center, traveling an estimated 150-250 miles weekly. The role involves providing services in accordance with the physicians plan of care, supervising therapy assistants, and coordinating discharge and final assessments with supervision. The contractor must have reliable transportation and will report to a designated location in Baton Rouge, LA.
