Malone Healthcare - Nursing is seeking a travel Physical Therapist for a travel job in Wisconsin Rapids, Wisconsin.

Shift: 8 hours, days
Employment Type: Travel

Shift Requirements: 8 variable days (Monday- Friday)
Float Requirements: occasional travel to central/ Northern regions
Mileage reimbursed at government approved rate
On Call/Call-Back: Can be low censused and put on call. Call back minimum is 2 hours
Weekends: PT's rotate one Saturday every 6-8 weeks. (will be given a a day off during the week on the weeks they work Saturday)

Holidays: None

Required Certifications: AHA or ARC BLS, Licensure required for the State of Wisconsin, Valid Drivers License (with proof of car insurance), NPI # required. If the candidate does not have one, they need to start the process of applying for a NPI # in order to start.

Required Education: Bachelor's Degree in Physical Therapy
Minimum Experience: Home Health experience, training working with the adult/geriatric population with a variety of orthopedic, neurological and functional mobility diagnosis. Knowledge of Physical Therapy standards and practices acquired through a minimum of a Masters degree in Physical Therapy.

Charting/Equipment: Laptop for point of service documentation, NetSmart EMR, and cell phone for GPS and communication.
