QMAP CNA – Group Home - CNA 0515 RR#01
Location: Steamboat Springs, CO
Schedule:
- Saturday 7:00 AM – Monday 3:00 PM
- Combination of 8-hour and 16-hour shifts
- 40 hours per week (5×8 schedule)

Position Summary
We are seeking compassionate and dependable QMAP CNA professionals to provide direct care and support within a residential group home setting.
This role combines Certified Nursing Assistant (CNA) responsibilities with Qualified Medication Administration Personnel (QMAP) duties, focusing on individualized care, daily living support, and community integration.
The ideal candidate is comfortable working in a home-like environment, supporting residents with daily activities, medication administration, transportation, and personalized care.
Key Responsibilities
Resident Care & Support- Provide direct care and assist residents with activities of daily living (ADLs)
- Support residents with daily routines, hygiene, and mobility
- Monitor resident conditions and report any changes or concerns
- Administer medications as a QMAP in accordance with state regulations
- Ensure accurate documentation and adherence to medication protocols
- Assist residents with:
- Appointments
- Community outings
- Recreational activities
- Support meal planning, grocery shopping, and household tasks
- Promote independence and community involvement
- Transport residents using a company-provided vehicle
- Ensure safe and reliable travel for appointments and activities
- Maintain a safe, clean, and supportive home environment
- Document care provided in accordance with facility policies
- Respond appropriately to overnight needs and emergencies
- Deliver compassionate, resident-centered care
- Build positive relationships with residents and team members
- Active CNA certification (required)
- QMAP certification (preferred or willingness to obtain)
- Ability to pass DMV background screening
- Clean driving record (preferred; no major violations or recent DUI)
- Strong communication and interpersonal skills
- Experience in:
- Group home
- Long-term care
- Residential care settings (preferred)
- Overnight stay required during scheduled rotations
- Interrupted sleep/work time compensated per facility policy
- Company vehicle provided for resident transportation
- Meals provided during resident outings and activities (as applicable)
- Colorado QMAP certification can be completed online
- Typically takes 5–10 days to obtain
