The QA RN will lead the development and implementation of the quality plan. Responsible for the quality review of clinical records and process metrics to ensure compliance and promote the highest level of clinical practice. Provides coaching and education based on findings to ensure optimal clinical practice competency and facilitate performance improvement.
Duties/Responsibilities:- Develops and leads the agency QAPI program. Chairs the QAPI leadership team.
- Gathers and collects clinical data for QAPI analysis.
- Oversees and monitors the actions of QAPI teams, ensuring performance improvement plans are developed and executed. Provides consultation to teams.
- Develops reports for the Professional Advisory Council.
- Develops and implement monitoring processes to ensure compliance with agency policy, professional standards, and state and federal regulations.
- Conducts ongoing clinical record audits aligned with the quality plan and identified areas of risk. Reports same to the QAPI Leadership team.
- Collaborates in providing strategy related to clinical practices and enhancing the patient experience.
- Collaborates with clinical leadership on developing and monitor quality goals.
- Monitors result against performance targets for key metrics and meet with Executive Director of Clinical Operations to review findings and provide recommendations for improvement.
- Ensures compliance with key metrics for all workflows, including but not limited to missed visits, timeliness of documentation, and appropriate Nursing and Therapy supervisory visits based on regulatory metrics.
- Works with clinical management to follow up on all patient Incident reports. Tracks and incorporates reporting in the QAPI process to identify ways to reduce risk.
- Collaborates with the Manager of Education and Director of Corporate Compliance to identify training needs and activities for staff that focus on accurate and appropriate evidence-based practice principles, existing tools and guidelines, and regulatory principles.
- Monitors variances/discrepancies to metrics with clinical management and provides insight as needed. Works collaboratively to find solutions to improve performance.
- Performs as a Clinical Manager on assigned weekends as part of a rotation with other managers.
- Provides backup to Clinical Managers and clinical workflow as necessary.
- Participates in special projects and performs other duties as required
- NYS RN License
- Prior experience in Quality Assurance
- Home Health experience (Preferred)
- Drivers License / Reliable Transportation
