Job Details- Weekly Estimated Pay: $2,500 – $2,630 (Competitive Travel RN Pay)

- Duration: 17 weeks
- Guaranteed Hours: 40 hours per week
- Shift: 8-hour days (08:00 AM – 04:30 PM)
- Weekend Rotation: Once every 6 weeks
- Number of Positions: 2 Travel RNs
- Minimum 1 year of certified home care / home health nursing experience
- Minimum 1 year of case management experience (discharge planning, care coordination)
- Current Registered Nurse (RN) licensure in the state of Massachusetts
- Reliable, independent means of transportation for home visits (valid driver’s license and vehicle)
- Willingness to work weekends, holidays, and participate in on-call rotations as needed
- Ability to provide clinical services to patients of all ages (pediatrics through geriatrics)
- Strong clinical assessment, documentation (EHR), and interdisciplinary communication skills
- Provide home health clinical services and home visits to patients from newborns to older adults
- Perform comprehensive home health assessments and develop individualized care plans
- Deliver patient and family education, disease management, and support for care at home
- Manage admissions, coordinate referrals, and perform case management duties in accordance with agency and state healthcare standards
- Maintain accurate, timely electronic documentation in the EHR and communicate effectively with the interdisciplinary care team
- Participate in weekend, holiday, and on-call rotations as required by the travel assignment
