Location: Anchorage, Alaska

We are seeking a qualified Medical Social Worker (LMSW or LCSW) to support inpatient case management in an acute care hospital setting. The role focuses on discharge planning, care coordination, and ensuring smooth transitions for patients across the continuum of care.

Key Responsibilities:

• Provide inpatient case management and discharge planning services
• Collaborate with interdisciplinary teams to coordinate patient care
• Assess psychosocial needs and develop appropriate care plans
• Facilitate safe and timely patient discharges
• Communicate effectively with patients, families, and care providers
• Maintain accurate documentation using EMR systems

Requirements:

• Minimum 1 year of acute care social work or case management experience
• Active Alaska LMSW or LCSW license required (pending license accepted)
• Strong experience in discharge planning and care coordination
• First-time travelers accepted
• Must be willing to work weekends as needed
• Must be willing to float within scope based on facility needs
