• Unit: 32-bed Transitional Care Unit
• EMR: Meditech
• Scrubs: Navy
• Float: 2N, 3N, 4N, and Rehab
• Charge experience: Preferred; charge nurse may be responsible for the whole unit depending on census.
• Weekend: Required
• Holidays: Required

• TCU: 1:8–10
• 2N/3N/4N: 1:6 max
• Rehab: 1:3–5
• Current practice: census around 18 = ~9 patients per nurse; at higher census, typically 2 nurses on nights.

Patient Population / Skills

Post-acute and step-down patients, commonly:

• Orthopedic surgery, especially hip/knee
• Various non-acute patients requiring skilled care
• Respiratory care/interventions
• Wound dressing/redressing
• IV antibiotics
• Scheduled therapy

No: vents, drips, telemetry, or strip interpretation.

• 2–3 aides on days/evenings; 1–2 nights depending on census
• Hospitalist available
• Pharmacy 24/7
• No IV team
• No 24/7 RT
• One RN float may assist with admissions, discharges, IVs, etc.
